The Ohio Issue 4 was on the November 4, 1975 ballot in Ohio as an initiated constitutional amendment, where it was defeated.
Issue 4 was a measure to authorize the issuance of bonds and notes in an amount not to exceed $2.75 million with the principal and interest to be paid by an additional levy of a 7/10ths of 1% sales and use tax.
